Quick Form
Collect structured information with a multi-field form.
What is Quick Form?
A Quick Form collects several pieces of information at once through a custom form with multiple fields — names, emails, choices and more. Useful for registration, lead capture or structured surveys.

Create a Quick Form slide
- 1On Home, click + Create new presentation (or add a slide to an existing one).
- 2In Choose Slide Type, pick Quick Form. You can also type it into Search slide types….
- 3The slide is created and the right panel opens on the Content tab.
Set up the content
- 1Add the fields you need (text, email, choice, etc.).
- 2Mark any fields as required.
- 3Give the form a clear title and instructions.

Add and reorder as many fields as you need.
Required fields
Force participants to complete key fields before submitting.
Time limit
Give the audience a countdown to respond. Turn on and set the number of seconds.
Hide result
Keep results hidden while people respond, then reveal them when you are ready.
Stop submission
Lock the slide so no more responses are accepted.
Present & share the join code
- 1Click Present in the top bar to put the slide on the big screen.
- 2A join code and a QR code appear (e.g.
36Y56). - 3Participants open the Audience app at join.engageslide.com, type the code and press Enter — or simply scan the QR.
- 4When everyone has joined, the responses start flowing in live.
How the audience responds
Participants fill in each field and submit the form once.
View results
All submissions are collected in a table you can review live and export for follow-up.
